Step #2 – Gather documentation required for registration

Registration Requirements
(Three (3) Proofs of Residency Required; see below)

• Living Arrangement:
  • Owned residence (www.scpafl.org) OR
  • Current lease or rental agreement (Address MUST match on all documents)
• Current utility bill (water or electric) or initial order for service
• One of the following three
  • Driver's license OR
  • Auto registration OR
  • Voter's registration card

***Address MUST match on ALL Documents***

Student Records Needed:

1. Withdrawal Form (previous school)
2. Transcripts: current grades or other academic history
3. Florida Certificate of Immunization (Form 680) - available from any health office or physician in State of Florida. Seminole County Health Department - 400 W. Airport Blvd, Sanford, FL., 407- 665- 3000.
4. Birth Certificate
5. Physical (First Time entering Florida public school) - physical exam must be completed within one year prior to enrollment. Health form must be signed and dated by physician and who results of examination. Cannot be a simple statement from a physician.
6. If applicable - current IEP/EP, Psychological/Placement Papers
7. Extenuating circumstances not covered above may require additional documentation.
