| 2. | Original birth certificate, for age validation, birth certificate will be returned. |
| 3. | Verification of Health Insurance for sports other than football. |
| 4. | Promoted from 8th grade, not assigned, if you are a freshman. |
| 5. | Have an unweighted cumulative grade point average of 2.0 or above. |
